It was established on June 28, 1941. During the existence of this award, its statute was changed twice - in 1943 and 1953.
The medal is awarded to enlisted personnel for faithful and honest service and exemplary behavior during service in the US Army.
The medal is a bronze disc 1 1/4 inches in diameter. The obverse depicts an eagle spreading its wings, sitting on a closed book. In a circle the entry "FIDELITE, HONOUR, EFFICIENCY".
Burgundy ribbon with white triple stripes around the edges.
Repeated awarding of the cluster on the ribbon and the bar of the medal in the form of a stylized rope with knots. The number of nodes indicates the number of rewards. The cluster is bronze for awards from the 2nd to the 5th, silver for awards from the 6th to the 10th, gold for awards from the 11th to the 15th.
